We read with interest the JOURNAL article concerning multiple distal anastomoses with internal mammary artery conduits
1 Although we agree with the authors' conclusions, we would like to point out that in 1985 we published our experience with a single patient undergoing five distal anastomoses on a single internal mammary artery conduit with an in situ and free sequential grafting technique.
2 In this communication we supplied a description, a schematic drawing of the operative technique used, and a frame from a postoperative cineangiogram depicting filling of all five distal coronary vessels. Although the techniques are somewhat different, the potential for this type of multiple-vessel revascularization with arterial conduit is underscored.
We would like to compliment the group on their large and excellent experience with multiple-vessel arterial revascularization. We emphatically support their conclusion that multivessel internal mammary artery grafting is not only feasible but desirable and can be safely used.
