Is an increase in neutrophil leukotriene-generating capacity a specific phenomenon for cardiopulmonary bypass?

We read with interest the article titled "Neutrophil Leukotriene Generation Increases After Cardiopulmonary Bypass" by Gadaleta and associatesGo 1 in the October 1994 issue of the Journal. They showed an increased neutrophil leukotriene production during cardiopulmonary bypass (CPB) and speculated a relationship between neutrophil-derived leukotriene and inflammatory response after CPB. Using the same method (neutrophil separation from patients' blood, activation with calcium ionophore A23187, leukotriene quatification by reversed-phase high-pressure liquid chromatography), weGo 2 reported the postoperative changes in neutrophil leukotriene productivity after CPB in 1988. In our study, increase in neutrophil leukotriene B4 production and decrease in leukotriene C4 production were reported.

In 1989, weGo 3 developed a hypothesis that the change in neutrophil functions might be related to and specific for. CPB Perioperative neutrophils were studied in 20 patients having cardiac operations with CPB (n = 10) and other major operations (n = 10). The increased leukotriene B4-producing capacity and decreased leukotriene C4 production were similarly observed in the patients of both groups.Go 3 Therefore we concluded that the leukotriene productivity of neutrophils is altered during CPB, but this change is not specific for the type of operation with CPB. Unfortunately, Gadaleta and associatesGo 1 did not have a control study group to allow the specificity for CPB to be discussed.
