Partial sternotomy for cardiac operations

Reply to the Editor:

I congratulate Walterbusch for his extensive experience with the partial sternotomy. His observations regarding patient selection and results are similar to my observations with the mini-T sternotomy.

Interestingly, without being aware of his work, I recently started angling the transverse cut on the sternum more like a "mini arrow," with the same idea of minimizing horizontal motion after closure. I have also started placing the vertical wires between the manubrium and the third intercostal spaces instead of the body of the sternum, as shown by the illustration in my article. This seems to provide a more stable closure, especially in patients with a thin sternum.
