• Video - This video illustrates a simplified Cox-Maze procedure using bipolar radiofrequency energy as an ablative energy source to replace most of the surgical incisions. The patient is a 35-year-old man with a 3-year-history of paroxysmal atrial fibrillation. He first presented with congestive heart failure and an ejection fraction (EF) of 10%. He failed trial of both amiodarone and sotalol and underwent a four-vessel pulmonary vein ablation in June 2003, which was unsuccessful. Hence he was referred for a surgical Maze procedure. His preoperative echocardiogram showed an EF of 25% and left atrial diameter of 4.1 cm. The patient’s cardiac catheterization revealed normal coronary arteries and an EF of 20%.
